Join an existing PBSG Group
If you know of a group in your area and the PBSG Facilitator has agreed for you to join the group, you can complete the registration form here and mail or fax it to us, or you can contact us at 1.800.661.3249, Option 1, and arrange registration and payment with us.
Once you have registered and paid, a Welcome Package (with modules) is mailed to your facilitator which you will receive at your first official meeting as part of a Practice Based Small Group.
Form a New PBSG Group
If you would like to form your own group, the recommended group size is seven members (with a minimum of four and a maximum of 10) including an appointed facilitator who will need to attend a one day workshop. In order to proceed, complete the Form a New Group contact form with your contact information so we can contact you and provide further information on how to get you started or you can call direct at 1.800.661.3249, Option 1 (Mon to Fri 8:00 am to 4:00 pm EST)
• Member Registration: Each new member (including facilitator) of the group completes a registration form with contact and payment information which can be returned individually, or as a group via fax or mail
• PBSG Facilitator Training: The facilitator must complete the workshop registration form with payment information and return it with the member registration forms via fax or mail. Important Note: In thanks for taking on the facilitator role, once the facilitator has been trained, their yearly membership fee will be waived for as long as they remain an active facilitator.
• Welcome Group Package: Once all group member payments are processed, we mail the facilitator a welcome group package that will include a PBSG Facilitator guide, role and responsibility of the facilitator, members online information, as well as modules for each group member. Prior to the first meeting the group should determine a mutually agreeable time and place to meet. Groups typically meet for 1-2 hours once a month, however, the schedule is entirely up to the group and what works best for member’s busy schedules (see CME-CPD Credit page for credit information).
